Tax Document Checklist

Below you will find examples of forms companies and individuals may have provided you with for tax purposes. If you have been provided with any of the following documents, 9 times out of 10 this information has already been reported to the IRS and should be included on your tax return! We have also given some examples of other documents and forms that may benefit you and should also be reported to the IRS. At the bottom of the page you will find a IRS Form Resource that will give you a detailed account of all the forms on this page and many more. 

Form W2

Employment Wage Form

Every employer engaged in a trade or business who pays remuneration, including noncash payments of $600 or more for the year (all amounts if any income, social security, or Medicare tax was withheld) for services performed by an employee must file a Form W-2 for each employee (even if the employee is related to the employer) from whom:

  • Income, Social Security, or Medicare tax was withheld.

  • Income tax would have been withheld if the employee had claimed no more than one withholding allowance or had not claimed exemption from withholding on Form W-4, Employee's Withholding Allowance Certificate.

1099NEC

Non-Employee Compensation

Given to contractors & self employed individuals and reported to the IRS. This form is used by companies and individuals to show proof of  wage type expenses paid to contractors.

1099Misc

Miscellaneous Income

File Form 1099-MISC for each person to whom you have paid during the year:

  • At least $10 in royalties or broker payments in lieu of dividends or tax-exempt interest.

  • At least $600 in:

    • Rents.

    • Prizes and awards.

    • Other income payments.

    • Medical and health care payments.

    • Crop insurance proceeds.

    • Cash payments for fish (or other aquatic life) you purchase from anyone engaged in the trade or business of catching fish.

    • Generally, the cash paid from a notional principal contract to an individual, partnership, or estate.

    • Payments to an attorney.

    • Any fishing boat proceeds.

In addition, use Form 1099-MISC to report that you made direct sales of at least $5,000 of consumer products to a buyer for resale anywhere other than a permanent retail establishment.

1099B

Proceeds from Broker and Barter Exchange Transactions

A broker or barter exchange must file this form for each person:

  • For whom, they sold stocks, commodities, regulated futures contracts, foreign currency contracts, forward contracts, debt instruments, options, securities futures contracts, etc., for cash,

  • Who received cash, stock, or other property from a corporation that the broker knows or has reason to know has had its stock acquired in an acquisition of control or had a substantial change in capital structure reportable on Form 8806, or

  • Who exchanged property or services through a barter exchange.

1099DIV

Dividends and Distributions

Form 1099-DIV is used by banks and other financial institutions to report dividends and other distributions to taxpayers and to the IRS.

1099G

Certain Government Payments

Federal, state, or local governments file this form if they made payments of:

  • Unemployment compensation.

  • State or local income tax refunds, credits, or offsets.

  • Reemployment trade adjustment assistance (RTAA) payments.

  • Taxable grants.

  • Agricultural payments.

They also file this form if they received payments on a Commodity Credit Corporation (CCC) loan.

1099H

Health Coverage Tax Credit (HCTC) Advance Payments

File this form if you received any advance payments during the calendar year of qualified health insurance payments for the benefit of eligible trade adjustment assistance (TAA), Reemployment TAA, or Pension Benefit Guaranty Corporation (PBGC) pension recipients and their qualifying family members.

1099INT

Interest Income

File Form 1099-INT for each person:

  • To whom you paid amounts reportable in boxes 1, 3, and 8 of at least $10.

  • For whom you withheld and paid any foreign tax on interest.

  • From whom you withheld (and did not refund) any federal income tax under the backup withholding rules regardless of the amount of the payment.

1099LTC

Long Term Care and Accelerated Death Benefits

File this form if you pay any long-term care benefits, including accelerated death benefits. Payers include insurance companies, governmental units, and viatical settlement providers.

1099Q

Payments from Qualified Education Programs (Under Sections 529 and 530)

File Form 1099-Q (Info Copy Only) if you:

  • Are an officer or an employee, or the designee of an officer or employee, having control of a program established by a state or eligible educational institution.

  • Made a distribution from a qualified tuition program (QTP).

1099R

Distributions From Pensions, Annuities, Retirement or Profit-Sharing Plans, IRAs, Insurance Contracts, etc.

File Form 1099-R for each person to whom you have made a designated distribution or are treated as having made a distribution of $10 or more from:

  • Profit-sharing or retirement plans.

  • Any individual retirement arrangements (IRAs).

  • Annuities, pensions, insurance contracts, survivor income benefit plans.

  • Permanent and total disability payments under life insurance contracts.

  • Charitable gift annuities, etc.

1099S

Use Form 1099-S to report the sale or exchange of real estate.
